Nurse practitioner documentation
An NP note has to show the whole episode of care.
You take the history, examine, order and interpret tests, start or change a medicine, refer and own the follow-up. Hanah drafts each part of that, and every clinical fact comes only from the consult. A section with nothing to put in it is left out.
Start on the NP templates Hanah ships with, or bring the ones your practice already uses. The wording is yours, and Hanah drafts into it.
Consultation note
The note runs from history and examination to your working diagnosis, what you prescribed or ordered, who you referred to and when you'll review the patient. Hanah drafts it as SOAP in shorthand by default.
Medication changes
Hanah drafts the medicine, dose, route and frequency, and what changed and why, from what was said in the room. Nothing is inferred, and old values from an earlier note aren't carried forward.
Results and safety-netting
The investigations you ordered and the results you went through go into the note. The safety-netting advice you gave about what to watch for and when to come back goes in as you said it.
Referral letter
The letter is addressed to the recipient and gives the reason for referral, the history, your findings and working diagnosis. It closes with the management you've started and what you're asking of them.
A brief before each review
Before a review, Hanah can brief you on the patient from their recent Hanah visits. If you use Cliniko, it also draws on their recent treatment notes.
The patient's care plan
The plan sets out identified needs, planned interventions, what's being monitored and when it will be reviewed. Each care task the patient does between visits gets its own card, and patients who prefer another language read a translated copy.

What Hanah is, in plain terms.
What notes does Hanah write for nurse practitioners?
A nurse practitioner starts with a consultation note in SOAP format and a referral letter. You can edit these or build your own, such as a wound care note or a medication review.
Can patients see their care plan?
Yes, if you use patient engagement. The nurse practitioner care plan sets out identified needs, interventions and how outcomes are monitored, with a card for each care task the patient or their family carries out between visits: what to do, how often and what to watch for. Patient engagement is optional; the notes work without it.
What is Hanah?
Hanah is a platform for modern health practices, and it starts with an AI scribe. The scribe drafts your notes, letters and referrals from the consult, and it runs standalone in your web browser or on iPhone, iPad and Android, with nothing to migrate. It doesn't need a practice management system or the browser extension. If your practice uses Cliniko, Halaxy, Nookal or Zanda, the optional Hanah browser extension puts the scribe inside it and sends each note to the right patient record. Care plans and a patient app that keep patients on plan between visits, and analytics across the practice, are there when you want them. You don't need either to use the scribe.
Is Hanah an AI scribe?
Yes. Hanah's AI scribe drafts your notes, referrals and care plans from the consult, in your own voice, and it can be the only part of Hanah you use. It runs inside Cliniko, Halaxy, Nookal and Zanda through the browser extension, or standalone if you don't use a practice management system. Patient engagement and practice analytics are in the same account if you want them later, but the scribe doesn't depend on them.
Does Hanah make diagnoses or treatment decisions?
No. Hanah helps you find and summarise information in existing notes and documents, and draft records of the findings, diagnoses and plans you provide. It supports your search for evidence within those materials and your review of the sources; it does not independently diagnose, choose treatment or decide what to investigate next. You assess the evidence, make the clinical decisions and check every draft against its sources.
Can I use just the scribe?
Yes. Record the consult or dictate afterwards, review the draft, and send the note to your practice management system or keep it in Hanah. You don't have to set up care plans, invite patients or use the analytics, and patients aren't sent anything unless you choose to send it. If you want patient engagement later, it's in the same account, with nothing to move across.
Who is Hanah for?
Hanah is built for allied health, GP, and specialist practices — from solo practitioners to multi-clinician groups — and the owners, managers, and clinicians who run them.
Where is my data stored, and is Hanah secure?
Patient records are stored in Australia, the UK or the US. New Zealand practices are served from Australia, and we also serve practices in Europe. Our data processing agreement lists which regions each step happens in. Bank-level encryption, and no model training on patient data. The regional Trust Centers list the full controls.
How quickly can a practice get started?
About 10 minutes to onboard each clinician. Most practices have the scribe live in week one and patient engagement switched on by week two.
