Occupational therapy documentation

An OT assessment is about what someone does at home.

Occupational therapy happens in a client's kitchen, beside a shower with no grab rails, or in the car after the visit. The documents that follow are about function, meaning what the client finds hard at home and the equipment or modifications that would help. Hanah drafts each of them from the visit into your own template or the funder's form.

Start on the OT templates Hanah ships with, or bring the ones your practice already uses. The wording is yours, and Hanah drafts into it.

Assessments and progress notes

The note opens with the client's own account of what's hard. What you observed across ADLs, IADLs and transfers follows, one finding to a line, and anything you didn't assess stays out.

NDIS reports

Bring your practice's report or the NDIA's Word template and Hanah fills it from one session or several. Goals stay in the participant's own words when you quote them, and a field the session gave no figure for is left blank.

Home modifications and assistive technology

Hanah drafts a home safety assessment, modification report or AT prescription from what you found and measured in the home. Photos you add to the visit, such as the bathroom you assessed, can go into a Word report.

Functional capacity and insurer forms

Upload the Word or fillable PDF form a funder or insurer gives you. Hanah ticks a checkbox only when the session clearly supports it and leaves signature fields for you.

Letters to the GP and referrer

Hanah drafts a referral or progress letter in full prose from the same visit as the note.

Care plans for the client

Each strategy, home change and piece of equipment you recommended gets a card saying what it helps with and when in the day it fits. Film a transfer technique on your phone to add it as a card.

How it works
  1. Step 1 Information assistant

    Walk in already prepared

Last session's plan, open goals, recent PROM scores, and the follow-up actions you recorded — brought together for your review.

  2. Step 2 AI Scribe

    Press record, stay present

Every detail captured live, in any language, even mid-sentence code-switching.

  3. Step 3 Home programs & outcomes

Capture the plan as you go

Hanah drafts the care plan you agreed with your patient, using the goals, activities and follow-up you stated in the consultation.

  4. Step 4 Documentation

    The rest writes itself

Draft your documents, forms and letters from the findings and decisions you stated, ready for your review.

  5. Step 5 Patient Health Literacy

Patients stay on plan between visits

Informed and kept in the loop, with communication scanned for jargon and explained — an informed patient is an engaged patient!

  • Sits inside your PMS in the browser — you never leave the patient you're on
  • Notes go into the correct patient record, in your own template
  • Two-way sync where your system offers an API — connect in minutes with a secure key
  • Patient history stays where it is — your PMS remains the source of truth

On a different system? Hanah still runs — notes, patient engagement and insights all work standalone while we build your integration. Tell us which system you use →

Common questions

What Hanah is, in plain terms.

What notes does Hanah write for occupational therapists?

An occupational therapist starts with an initial assessment, a progress note and a discharge summary in SOAP format, plus a referral letter. You can edit these or build your own, such as a home safety assessment, or upload a Word or PDF report template you already use.

Can patients see their strategies at home?

Yes, if you use patient engagement. The OT care plan covers what you're working on, the strategies and equipment you recommended, and day-to-day practice, with a card for each activity, adaptation or piece of equipment: what to do, when in the day it fits, and which everyday task it makes easier. Patient engagement is optional; the notes work without it.

What is Hanah?

Hanah is a platform for modern health practices, and it starts with an AI scribe. The scribe drafts your notes, letters and referrals from the consult, and it runs standalone in your web browser or on iPhone, iPad and Android, with nothing to migrate. It doesn't need a practice management system or the browser extension. If your practice uses Cliniko, Halaxy, Nookal or Zanda, the optional Hanah browser extension puts the scribe inside it and sends each note to the right patient record. Care plans and a patient app that keep patients on plan between visits, and analytics across the practice, are there when you want them. You don't need either to use the scribe.

Is Hanah an AI scribe?

Yes. Hanah's AI scribe drafts your notes, referrals and care plans from the consult, in your own voice, and it can be the only part of Hanah you use. It runs inside Cliniko, Halaxy, Nookal and Zanda through the browser extension, or standalone if you don't use a practice management system. Patient engagement and practice analytics are in the same account if you want them later, but the scribe doesn't depend on them.

Does Hanah make diagnoses or treatment decisions?

No. Hanah helps you find and summarise information in existing notes and documents, and draft records of the findings, diagnoses and plans you provide. It supports your search for evidence within those materials and your review of the sources; it does not independently diagnose, choose treatment or decide what to investigate next. You assess the evidence, make the clinical decisions and check every draft against its sources.

Can I use just the scribe?

Yes. Record the consult or dictate afterwards, review the draft, and send the note to your practice management system or keep it in Hanah. You don't have to set up care plans, invite patients or use the analytics, and patients aren't sent anything unless you choose to send it. If you want patient engagement later, it's in the same account, with nothing to move across.

Who is Hanah for?

Hanah is built for allied health, GP, and specialist practices — from solo practitioners to multi-clinician groups — and the owners, managers, and clinicians who run them.

Where is my data stored, and is Hanah secure?

Patient records are stored in Australia, the UK or the US. New Zealand practices are served from Australia, and we also serve practices in Europe. Our data processing agreement lists which regions each step happens in. Bank-level encryption, and no model training on patient data. The regional Trust Centers list the full controls.

How quickly can a practice get started?

About 10 minutes to onboard each clinician. Most practices have the scribe live in week one and patient engagement switched on by week two.

See Hanah running your practice in 20 minutes.

We'll walk through the scribe, the patient app, and the practice dashboard on your actual workflow — not a sandbox.
