Psychology documentation
What matters clinically often sits under the words, in the avoidance or the topic the client keeps coming back to. Hanah drafts a neutral account of what the client said and did and relates it to your working formulation. It also records the between session tasks you agreed.
Start on the psychology templates Hanah ships with, or bring the ones your practice already uses. The wording is yours, and Hanah drafts into it.
Hanah drafts the presenting concerns, developmental and social history and your initial formulation in full prose by default.
Each session gets data, assessment and plan, kept factual and neutral. If you work in SOAP, BIRP or GIRP, Hanah fills those headings instead.
Add an MSE section to your template and Hanah fills it from what you observed and said, from appearance and behaviour through to insight and judgement. Anything you didn't comment on is left out.
Hanah drafts a letter back to the referring GP from the same session. At the end of a Better Access course of treatment, pick the sessions and Hanah drafts one letter from all of them.
The plan describes what you're working on together in language the client would recognise, the approach you're taking, and one card for each skill to practise before next time. Diagnostic labels stay out unless you discussed them openly.
A risk and safety section sits alongside the therapy plan and only you can see it. Hanah updates it when a later session changes it.
Last session's plan, open goals, recent PROM scores, and the follow-up actions you recorded — brought together for your review.
Every detail captured live, in any language, even mid-sentence code-switching.
Hanah drafts the care plan you agreed with your patient, using the goals, activities and follow-up you stated in the consultation.
Draft your documents, forms and letters from the findings and decisions you stated, ready for your review.
Informed and kept in the loop, with communication scanned for jargon and explained — an informed patient is an engaged patient!
We worked on spotting cognitive distortionsCognitive distortions — unhelpful thinking habits, like expecting the worst, that make things feel harder than they are.. Your therapist’s plan for this week is to use a thought recordThought record — a short worksheet for noting what happened, what you thought and how you felt, so the pattern is easier to see. after a tough moment, and next session we’ll start behavioural activationBehavioural activation — planning small, enjoyable or meaningful activities to lift your mood, even before you feel like doing them..
When the plan lives in their pocket, follow-ups get booked, not forgotten — the loop that turns a good visit into a lasting outcome.

Yes. A psychologist starts with an intake assessment and a progress note in DAP format (Data, Assessment, Plan) plus a referral letter. Data stays factual and neutral, Assessment relates the session to your working formulation, and Plan covers ongoing treatment, between-session tasks and review.
Yes. The therapy plan you can share with a client has a clinical risk and safety plan section marked clinician-only, so it stays out of the client's view. The rest of the plan (what you're working on, your approach, and skills to practise between sessions) is written in plain, non-judgemental language. Sharing a plan is optional; DAP notes work without it.
Yes. You can edit the starter templates or build your own, such as a mental state examination, a risk assessment or a CBT session note, or upload a Word or PDF report template you already use. Hanah fills whichever one you pick from the session.
Hanah is a platform for modern health practices, and it starts with an AI scribe. The scribe drafts your notes, letters and referrals from the consult, and it runs standalone in your web browser or on iPhone, iPad and Android, with nothing to migrate. It doesn't need a practice management system or the browser extension. If your practice uses Cliniko, Halaxy, Nookal or Zanda, the optional Hanah browser extension puts the scribe inside it and sends each note to the right patient record. Care plans and a patient app that keep patients on plan between visits, and analytics across the practice, are there when you want them. You don't need either to use the scribe.
Yes. Hanah's AI scribe drafts your notes, referrals and care plans from the consult, in your own voice, and it can be the only part of Hanah you use. It runs inside Cliniko, Halaxy, Nookal and Zanda through the browser extension, or standalone if you don't use a practice management system. Patient engagement and practice analytics are in the same account if you want them later, but the scribe doesn't depend on them.
No. Hanah helps you find and summarise information in existing notes and documents, and draft records of the findings, diagnoses and plans you provide. It supports your search for evidence within those materials and your review of the sources; it does not independently diagnose, choose treatment or decide what to investigate next. You assess the evidence, make the clinical decisions and check every draft against its sources.
Yes. Record the consult or dictate afterwards, review the draft, and send the note to your practice management system or keep it in Hanah. You don't have to set up care plans, invite patients or use the analytics, and patients aren't sent anything unless you choose to send it. If you want patient engagement later, it's in the same account, with nothing to move across.
Hanah is built for allied health, GP, and specialist practices — from solo practitioners to multi-clinician groups — and the owners, managers, and clinicians who run them.
Patient records are stored in Australia, the UK or the US. New Zealand practices are served from Australia, and we also serve practices in Europe. Our data processing agreement lists which regions each step happens in. Bank-level encryption, and no model training on patient data. The regional Trust Centers list the full controls.
About 10 minutes to onboard each clinician. Most practices have the scribe live in week one and patient engagement switched on by week two.
We'll walk through the scribe, the patient app, and the practice dashboard on your actual workflow — not a sandbox.
